I think most of these were essentially virtual trip-wires. set up a ton of suspicious yet not quite illegal stuff faintly connected to the actually criminal organizations, and depending on things like website traffic, police interrogations, news coverage, etc you can determine who's on your tail and how much they know. at the first indication of anybody taking notice, you relocate your operation entirely, while the investigation into the now defunct cell continues on for years.

Also people tend to significantly overestimate how hard it is to create a criminal network - we know international trafficking of humans, drugs and organs happens, do you deny that?
People also tend to overestimate how many people have to be in on something. Take 9/11, a small handful of people within the US government probably new it was going to happen, and a small handful of other people in the states. Then a small handful of people in Saudi Arabia. Thats it. Not even that many people. Furthermore, how is the alternative really that difference? A conspiracy of a similar number of shitskins somewhere in the middle east? The biggest difference is that the former have far more power, influence and resources to make things happen and keep it hidden.
Then there is the other types of "hybrid" conspiracies. You get other people to do something or simply allow them to do it. They don't know your plans and don't even realize they are part of a conspiracy. Its as simple as encouraging and facilitating an unhinged individual to commit an act of terror. Or for example, Pearl Harbor. FOIA documents have shown us FDR had to have known it was coming. He just moved the new ships out and let it happen. The urgent memos sent simply never arrived. It is still a "conspiracy" but FDR himself didn't actually do very much to facilitate it, just didn't stop it.
Back to pedophilia. Consider this. In every state in the US, and in a lot of other developed nations. If you are so inclined, you can access pretty much any drug you want, illegal weapons, and sex slaves/prostitutes. That is simply reality. Shady organizations are constantly working in the background all around us and have been for centuries. Don't be naive about that.

They dont have to. Usually if you are in the group you are benefitting and onboard. You have little incentive to speak out and implicate yourself. Lets not forget that these are highly illegal and reprehensible actions. People hold far less secrets their entire lives. Why would someone who agrees to a conspiracy then out themselves?

And finally, there are repercussions for people who get too close or try to speak out - usually death. Look at the deaths of Michael Hastings and Danny Casolaro. Or the numerous polonium poisonings from the Russians. Or the long list of deaths connected to the Clintons. People get killed for much less, and the more powerful you are the easier it is to cover it up. Maybe a brave soul might decide to defect, even if he can survive, who would believe him? How far will he go to incriminate himself?
Think this through user. We know for a fact that all kinds of illegal crime rings and conspiracies have existed. How many people spoke out about them? It is extremely rare that people will, for all the reasons above. I mean christ user, do you realize how hard it is for police to even get a witness to testify for a crime? Even if they aren't involved. Imagine on a much larger scale where the witness protection program isn't even an option not too mention you know your guilty too and people will likely not even believe you anyways.
