It turns out he wasn't isekai'd, just had braindamage and percives the world drastically different. Ends with MC put in an asylum, or killing himself.

It's not a different world, but the past of his own world, and no one knew magic existed back then. Proceeds to actually end his own lineage and ends up in a paradox.

Fantasy characters get isekai'd to the modern world and have to adjust to normal society.

Or even better, is running off actual layman's understanding of actually relevant current day knowledge to that time and just consistently gets thing terribly wrong.

Actually that manga already exists as a post-apocalypse story called Survival

Has a battle harem isekai ever been done before?

Having several different viewpoints like twelve kingdoms did.

The isekai world is real but nobody gives a fuck and the MC dies in the 2ª episode because ""freaking weirdo with no magic like Us"", then the story ends and nobody remembers him/her.

As soon the main plot/demon lord is defeated the story continues when he is isekai'd back to his own world but there is an alien invasion or some shit going on.

No RPG mechanics and the setting is distinctly Japanese/East Asian, not Medieval Europe.
No special powers, but the MC awakens with a powerful weapon or artifact that they need to practice with to learn how to effectively use.
No heroine has a bust size smaller than D.

MC slowly becomes mad with power and becomes the demon lord.
In the sequel, a young knight, the new MC, defeats the demon lord and gets sent in his world, which is our world.
Goes mad with power and becomes a politician.

Hillary Clinton wins the elections and then... somehow, everything transformed into an isekai

The new world is actually diferent Disney worlds. The harem is all the Disney princesses and the bad guys are parodies of the princes.

God exists, it´s a cute little shota and transforms the world into a magic isekai because "Humans are too violent and that´s s-sad"
