it has be a combination of both. there's no simple way to execute it in a country the size of the us. a certain amount of people have to be paying into it and insurance companies can't extort people with the prices

How would that work? universal healthcare has to be paid for by someone. There's no hidden army of doctors, nurses, techs, and orderlies that would be ok with working probono.

Private healthcare is simply healthcare administered by parties other than the government.

Universal healthcare is a system that provides healthcare to all citizens of a given jurisdiction.

You can have healthcare that is administered privately but available to all citizens, cf. Medicare.

The important thing to remember in all this is that healthcare is not a right. Positive rights don't exist.
