Therapeutic Use Exemptions (TUE) are a thing, it's possible to take testosterone and still legally compete in elite sports. It's supposed to be highly regulated and difficult to get one, but of course they're abused whenever possible, e.g. it's how Lance Armstrong got started with his cheating.
Intersex != transgender. Intersex means your embryonic development fucked up and you're not fully male or female on a biological level (although you are genetically), e.g. you appear to be a woman but have undescended internal testicles.
