A number of things. Iran had a surge for nationalism (removing American/western culture) and instead of killing Khomeini, Reza Shah let him flee. Khomeini continued to broadcast anti-Shah/American/Western values, very much in the same way bin Laden and Qutb did.
Communists also aimed to remove the Shah in favor of a socialist regime over a monarchy.
If you want the truth, I believe the Shah was enacting reforms in his country because he was dying from cancer and similar to Obama rushing an Iran deal at the end of his tenure, Reza Shah wanted to be remembered as the Iranian ruler who modernized Iran with his White Revolution.
Conveniently, he died a year after the revolution. Because of his rapid reforms, the Iranian clerics/ayatollahs felt that their culture and government were moving too fast (imagine the United States legalizing public nudity...in the 1910s.)
I haven't read the exact facts to prove it, but I do believe one of the land reforms of the White Revolution involved breaking up private lands for public use, which would've diminished the wealth of the mullahs. If you look at Iran now, it's a system of corrupted officials and clerics. I believe that the reforms angered the wealthy class and they managed to secure their wealth by disposing of Shah, ironically working with the communists.
