It depends on what your end goal is, but for the most part, yes dirty bombs can be very effective.
They are ideal for causing a ridiculous amount of chaos and mass hysteria in a confined area like a large, compact, urban environment. The goal isn't to kill a whole bunch of people, it's to scare the shit out of them to the point that they no longer use reason or logic and therefore become desperate to get themselves and their families out of the zone of contamination. The authorities then loose control as the masses begin to panic and a state of anarchy is born. Now, under the guise of panic and chaos, you can rob banks, armored cars, ATMs, and apartments/houses. arm yourself with whatever is left in armories, police stations, gun shops, private gun collections, etc. Over run a federal building and take hostages, demanding money from authorities who are already stressed and spread thin. over run a manor outside the city and take hostages, and then make them and their friends give u their money. kill people who, before this incident, were much harder to get to.
Or pull a Soros and find some way to make money off all the chaos. Either way, cancer and radioactive poisoning soon becomes the least of your worries

maybe if you are standing next to the device and get splattered with radioactive material directly. people massively overestimate how much radiation random radioactive waste emits, and how dangerous it is.
this is a good point. it is definitely possible that it could trigger hysteria and mass panic.
depends of the size of the bomb and the scale of the attack though. even 9/11 didn't create mass panic and anarchy in new york, and that was a massive attack.

The thing is, normal people dont really understand how radioactive materials work, so side of the bomb barely matters. They just think of bombs and the nuclear fallout from Chernobyl and in Japan and it sends them into a panic. All they can think about is their child become deformed and they start to mentally break down. The effects of a dirty bomb are much more psychological than physical.
Also, 911 didn't create mass panic in the streets, but it did make everyone in the city extremely paranoid and on high alert for the next couple months. Honestly, all it would've taken was a couple more bombs strategically detonated around the city at random location of mass conglomeration to really scare the ever living fuck out of that ENTIRE city. Thank god they didn't.
