If ALL of these positions have been exhausted which would be hard because there is ALWAYS at least one "designated survivor" that will never be with the main group of these candidates if they are all in one location. The designated survivor will be set to an undisclosed location that is secure and physically distant from the rest of the group. If there are multiple designated survivors at any given time they will all be in entirely separate and physically distant locations from each other as well.

The 25th Amendment does not designate an "interim president" it designates the REPLACEMENT of the president. If the president becomes able to resume duties then they return to the position but if they don't the only way to remove the replacement is by impeachment.
