Why is there no middle ground between sexually impotent and a literal rapist in fantasy manga? Can mangaka really not comprehend how to naturally transition into 2 people having sex? In a lot of western stuff with heroes you have them generally having one-night stands with grateful peasant bitches or some rescued princess.

And this isn't exactly new either. I remember reading Bakuretsu Hunters and the MC, a self-proclaimed playboy and womanizer, spends the entire series trying to fuck the women he helps, only to be met with comedic foil from his 2 femdom bitch childhood friends. Eventually, near the end, he's finally about to stick his dick in a girl and chickens out at the last second saying "I have someone I like". He spent the entire fucking series trying to coerce the women he rescued into sex and pussies out at the end. He doesn't even have sex with the childhood friend that he chooses. The only time he gets laid is in the absent space between the original manga and the sequel.

It's particularly frustrating when the plot of the manga includes sex as a central component. Or when the characters are legally married and they still wait for some reason. It seems strange to be fighting life and death battles everyday and nobody brings up "I don't want to die a virgin".

Japanese mindset is pretty fucked up when it comes to this sort of things. Then again, US of A is still stuck with that puritan complex, so it's not the only country that seems to have a problem with casual boning.

Considering the best fantasy manga has a pretty well done romance, even with consensual sex in the missionary position that results in procreation, I don't really know what to tell you.
I mean, there's lots of rape in it, too, but there's a teensy bit of "normal" stuff here and there.

The reason it's relatively rare in modern fantasy manga is that most fantasy stories are isekai or shounen, which will basically never have explicit sex. It's just not part of the demographic. Seinen series tend to have more sexual content but, even then, it's not pretty common for characters to sleep around a whole ton - simply because Japanese literature doesn't focus on things in general very much. Many manga series also bank strongly on teasing and sexual tension, so those plotlines will not be resolved until the end of the story, because they keep the readers invested.

In Berserk, Casca and Guts have a sweet, consensual sex scene.

It's very rare to see two adult characters that are not in love and just happen to be friends, coworkers or acquaintances who enjoy each other's company and go out together to have sex once in a while.

though there was no real romantic sub-plots at all and the MC mostly just keeps a professional relationship with the girls he fucked.

No, see, they're quite different. For one, implications don't usually do the things mentioned that sex scenes do, and for another implications are often received very differently by audiences and more easily allowed/tolerated by semi-puritans.

There's really two pieces here - the one piece is sex happening, and the other is it being shown.

They both have the same frequency of nonoccurrence. There's really not much else to talk about. There are hardly any manga that utilize fade-to-blacks too.
