I remember hearing reports of torture a while ago, before this article. I'm thinking that whatever it was they gave him the US doesn't full disclosure on. My guess is they want to make it clear what NK did to him without creating panic.

They probably plastic bagged him and cut off his oxygen, until he went into a coma. Subsequent to the coma he contracted an infection in one of North Korea's dingy and grimy detention centers. The infection combined with the acute stress and poor diet probably overran his body's defenses.
I HIGHLY doubt he was beaten to a pulp or mutilated by the North Koreans. This happened before Trump started putting pressure on Kim.
