The art is just completely grotesque. Berserk's art can be the same way at times, but there, it's mostly intentional. Shigurui was hyperdetailed enough to hit that uncanny valley where everyone looks monstrous even when they aren't supposed to.

I love how Araki can show so much, there's no dialogue that goes to waste on monologues or explanations because it's all right in the art.
