They must compromise integrity and consistency. This is why you have Dória who claims to be a liberal but is against civilian gun ownership, and Bolsonaro who claims to be a conservative but wants to close the economy and nationalize the industries.

People responded well to Dória's public-private partnerships in São Paulo's health system. The mainstream idea is that the market is incapable of providing certain goods, and thus the government must step in and provide it itself. It is not absurd to think that a more "extremist" position such as a full privatization would resonate nicely with the public as a fresh alternative to decades and decades of socialized healthcare.

To create a new one is a pain, it involves getting hundreds of thousands of signatures in every state, even in the most insignificant ones. Given Brazil's demographic spread, it is perfectly possible for a majority of our population to subscribe to a set of ideals, but still be unable to create a new party founded on such ideals.
