If you shop around as a cash/non-insured customer a lot of doctors, specialists, and even labs will offer extremely low priced flat fees aside for major surgeries or procedures. I don't see why this wouldn't remain the way things were if there was no health insurance system - maybe a bit more but not suddenly 10000000000000000000000 instead of a couple hundred.
