I don't think too much detail is anything bad. Berserk usually does that to encapsulate the sheer massiveness or scope of its setting which I think it does greatly.
Berserk always uses this to show the imposing atmosphere.

This was one of the covers for the "Justice" arc. Technically, Hol Horse helped the Crusaders by telling Jotaro about Justice and how it works.
A LOT of people speculated that Hol Horse was supposed to join the Crusaders at some point because of this cover, and that Araki being Araki dropped the idea last minute.

Asano does a lot of stuff with photos, compositing and drawing over them and stuff. It's noticeable in the bottom right of

Depends on the artist. Oyasumi punpun uses a lot of photos turned black and white. Murata and Miura hand-draw basically everything. Lots of others trace photos for backgrounds.
There's a video series on youtube of Kaoru Mori drawing an Otoyomegatari poster to give you an idea of the process.

Most of Inio Asano's work consists of actual pictures put through a filter, Copy & Paste techniques, and other sorts of digital manipulation. It's just that he does it all so well you barely notice.
Most traditional works though are just the product of a talented mangaka and their assistants
