The important context he doesn't mention here is that inbreeding (especially among cousins/distant relatives) was exceptionally common in every feudal society, including european ones.

It basically only started going out of style in the west as aristocracy fell and democracy rose, and its prevalence is mainly a practical consequence of feudal governments being obsessed with bloodlines and marriages while not understanding genetics. It only seems so weird to the US because, even as a colony, America never really had a feudal aristocracy to speak of, and thus no imperative for widespread incest in their historical conscious.

And no, 'rules' against it were never followed. There were no real rules for aristocracy beyond the ones they set themselves. The Habsburgs' level of consanguineous marriage was not typical for medieval or early modern Europe. It was far more common, especially before the 17th century, for European monarchs to marry people they weren't related to at all
