Yes, I was expecting a baby to just show up after a timeskip or something. The reason it didn't show them do anything prior to that was because of how they adapted Nagisa's route. In the game she and Tomoya start dating much earlier in the story (when Nagisa gives up on the drama club after she gets the threat letter), and their first kiss (pic related) takes place in that scene in season 1 when Tomoya goes on a picnic with her. But in the show they aren't dating yet at that point, so they couldn't include it. I guess once they missed their opportunity they just didn't bother to make a new one.

Thinking about it, I don't know where they could have put a kiss scene. Tomoya doesn't ask her out until the end of Season 1, and doing it there would be too quick. Maybe in the extra episode with Mei? After that the next episode that focuses on them is s2e9, but Nagisa gets sick there and by the time Tomoya graduates it should have already happened. Not related, but there are some anime-original scenes in After Story, e.g. Ushio wandering around and going to the hospital.
