She was alive, but was actually living in the afterlife the whole time. Kokkuri-san rescued her from a near-death experience by bringing her to the other side. Kohina, Kokkuri-san, Inugami, and Shigaraki were real, but literally everything else was an illusion. Most notably, Jimeko-san, but also characters like Kureha, Yamamoto-kun, Tama, as well as Noelle and her father. All fake as shit. Upon learning this revelation, Kohina smiles warmly and the manga ends.
As for behavior, yeah, she was just a weird kid.

Correction:
She smiled when Kokkuri-san and the other two came back to live with her in the real world, not when learning everyone else is fake. Her actual reaction to learning that everything else was fake was mostly indifferent. She only cared about the three ghosts that lived with her.
