Do they have AI that detect certain users and if they go onto certain websites? Do they detect certain words in comments?
And we always joke about being on "the list", but how true is it? Is everyone that visits certain sites automatically put on a list, and then only looked at by people if they cross a certain boundary?

True, but it's much more complicated than that. There are multiple systems with multiple indexes for each individual. Everyone's identity has its own records and multiple agencies are following your online and meatspace behavior for any actions/trends that might be significant. They rely heavily on exception reporting. That is, any break in normal trends in your behavior. Travel, weird new purchase etc. Similar to how credit card companies know that your card was stolen when you go out there and buy things that you normally wouldn't purchase. Profile.
