go to your facebook settings
click 'Download a copy of your data' they'll email it to you
once it's downloaded, you can open it to extract all the files out. then open the 'index.htm' file and go to the 'contact info' tab

There should be a ZIP file with your username in it. Extract that by right clicking, open the 'html' folder, open 'index.htm' in a web browser then navigate to the 'Contact Info' tab
