The forward-assist was added by the U.S. Army as an alternative to clearing a jam caused by carbon build-up/dirt. Build-up can cause a cartridge to stick in the chamber pert way. Thumping the forward-assist with your hand may force the cartridge all the in allowing it to fire, or doing so may make it worse. As far as build-up carbon goes, it is difficult to carry enough ammunition to do this, or be so hard pressed for time you can't clean your gun. It would probably take firing a couple of thousand rounds without cleaning to make carbon a risk. Dirt of course can come at anytime, but civilians can just stop and clean it, but a soldier in combat may not be able to, and the forward-assist option can be desirable.
The little door helps keep dirt out while being carried or stored. It opens automatically when the gun is fired.
