Interceptors are fired in pairs at a missle. In the case of the one fired at your capital the first interceptor hit and the second followed the debris trajectory. Since having a blast fragmentation warhead hit a populated area isn't preferable urban upturn mode was added. After a successful hit is made the remaining interceptors turn skyward and head to a safe height before detonating.

Could've been a lot of things. Software bug or sometimes if you leave the battery on too long it can cause a rounding error which leads to inaccuracy. I'm sure an internal review will be conducted by both the Saudi military and Raytheon.

Actually, SCUDs are notoriously difficult to shoot down precisely because of their obsolescence. In the Gulf War, some got through because they broke apart as they came down, creating multiple targets.
