Pic related is the entirety of the known Naruto world. We have no idea what lies to the West, North or South; there appears to be another continent to the East but we know nothing of it either. Are there any Europe, Africa or America equivalents in this Japan-inspired world? Are there countries where people can use Chakra but don't follow Shinobi or Feudal culture?

It's a typical trait of Japanese shows to have countries start and end within a day's march or so. Kino's Journey has fucktons of micro countries that make no sense, yet also look almost the same.
To the west are all of the actual, modern countries with the present day technology they use in some episodes. They're not really allowed to go there.
They use English words like "rival", "speed", "timing" or "rookie" all the time, so clearly there has to be some other place that speaks a language similar to English with enough cultural influence to seep into their world
Or at least, that's what logic would dictate. It could just be Kishi not putting any thought into the implications of his world.
Loan words can be very deceiving. Most likely the author didn't even think of them, just like most English speakers don't tend to care about the use of French originating terms, like pork and beef.
All the black people in the Hidden Cloud have to come from somewhere. Come to think of it, do they ever actually acknowledge ethnic differences? Nobody bats an eye upon seeing black people, even though real-world feudal Japan was pretty xenophobic.
But pork and beef aren't spelled in different set of characterst.

Japanese wasn't originally written with Chinese characters either. That's why the pronunciation and usage of Chinese characters is so much more complex and counterintuitive than in Chinese.

Japanese used to be written exclusively in Chinese. That's where they learned to write to begin with.
The reason Japanese kanji is so counter-intuitive, is because Japanese choose to spell Chinese loanwords the same as their native Japanese words.
Imagine if in English, we decided to spell "helix", "spiral" and "wind/wound" the same way, and still pronounced each one differently depending on when it's used.
This is what Japanese does.

The villages aren't that small, by the Boruto era they're more like full-fledged cities.

Konoha is huge, the red line is 20 km, the yellow line is around 60km. Konoha is a circle, and the very top of that circle is 120km across, or a little less than 75 miles.
