Because in most of these countries publisher has to pay money for a new examination. Why should they do it if game is old anyway and won't sell and there's still a chance to fail to get a rating.

The last of us sections where you have to kill humans are the closest thing we'll get to an actual Manhunt.

Does anyone else find Manhunt legitimately spooky? The music makes me uncomfortable.

I remember it being one of the first games to legitimately make me feel uncomfortable playing in some parts.
Especially early on, all of the victims seem so helpless, the kills excessively violent. The AI was easy enough to trick, so it was sometimes as easy as walking up and just mutilating them. It wasn't right. I loved it.
It scared me pretty bad on my first playthrough, especially the lunatic asylum. I was jaded to spooky games like Resident Evil or Silent Hill where you're fighting mutants or magic, but this snuff scenario got under my skin.
There was a kid who played manhunt in the towb next to mine and he ended up going on a killing spree, killed like 20 people with a plastic bag and shard of glass

Personally I found Manhunt 2 to be a lot more disturbing than the first. There's a farcical undertone to the first that is missing in the second.
Old banned games just get forgotten so even if newer games are more gruesome and still allowed nobody goes back to compare them and see if the ban is still warrented.
