The dash plugin is called Dash to Dock (Only on GNOME)
The desktop theme is along the lines of "OSX" or "Mac". It comes with GNOME
Under panel general properties
Set size to 48
Un-select expand
In background properties
Set colour to a solid dark grey
set style to whatever opacity you want
the rest are just icons for whatever programs you want
you can download and set them as you like
I have the classical windowlist and clock applet on the bottom panel and no top panel
And a similar panel like your picture on both sides
left side is all frequently used apps
right side is notification area, system tray,logoff,shutdown, lockscreen, etc.
