they don't really care if you can play one. you have to pass a written test that has to do with restoring pianos, materials, what not to use, and some tuning theory.
then there is the much more difficult tuning test where you have to tune a piano to spec without any tuning aids except an 1440 tuning fork, in front of a panel of "expert" tuners, who get to decide if your job was up to spec
you can take hours and hours if you want, but it has to produce a near perfectly tuned piano by ear

for some reason people tend to only get their pianos tuned by registered technicians dumb as hell if you ask me. its not THAT hard its not like its dangerous or anything, just takes lots of practice and I guess the cert. proves that you've done it before
