Well long term, if he's getting old and slowing down, I'd guess he and the Linux Foundation would designate a successor. If he drops dead suddenly, I'd guess one or several of his lieutenants would take over as the gate keeper.
The kernel is already pretty federated so he really only does high level merge work. It won't collapse without him. The lieutenants send him pull requests for the subsystems (net, storage, etc...). The lieutenants get their pull request from the maintainers of individual modules in the system (e.g. a specific driver). The module maintainers field the individual pull requests for new features and bug fixes.

I think Linus made git for many reasons, including this one. When he dies, anyone can voluntarily see every change he made, along with all his commit memos.
Combined with the mailing list, what more do we need from him? He's documented all his work. We are fine without him.

Linus has quite a few devs just below him that he considers highly trusted. 2nd, 3rd, 4th, etc in line to control the project basically. If Linus cannot continue to oversee the project for some reason then one of them will likely take over. Linus already pretty much relies on these devs to oversee sections of the kernel anyway. Linus mainly decides when its time to stop accepting features for a particular kernel release and he also decides when the kernel has gone through enough release candidates to be considered stable for a general release.
