What has me wondering are all those people in the mid east just reporting as "american".

They are not very aware of their heritage, but it mostly just means they are Scottish/Scots-Irish and sometimes English-Americans. But they have been there so long that they stopped caring, that and the fact that many who reported American often have black heritage they did not know about.
