It was a trait with a single rail for both directions; someone did not respect the precedence, human error.

There almost certainly is.

The track is divided into "blocks", when a train occupies a block, it shunts an electrical circuit that goes through the rail. The dispatcher should at a minimum know what blocks are occupied and the direction of travel.

It's almost certainly a dispatcher error, or one of the train drivers ignored/didn't see a red light.
