every time I've ever been asked "where are you from" and answered "USA" they've followed with "I know, but which state?"

England and Scotland are countries, California isn't.

Within the UK they will, and if they're speaking to foreigners outside of the UK the particularity nationalistic people will specify England, Scotland ect

because America is a large, diverse country and as far as they know I could be from Alaska, or Hawaii or Vermont, all of which are vastly different and in different time zones.
