256kbps is transparent with a good codec, AAC for example.
192/128 is totally acceptable with AAC and OPUS, especially when encoded straight from a lossless source (128 moreso with OPUS but still.)

Because everything has already been encoded in MP3 a long time ago. Converting it is detrimental to the quality no matter how good the new format is, so nobody does, and nobody has the source disks anymore also, or even wants to bother re-encoding them.

Now it is, but it didn't use to be. Even while it was technically a better format in the beginning, the quality optimizations in the MP3 (Lame) encoder were still years ahead. Also, support in hardware players was bad, so nobody bothered. It wasn't even popular in avi's because that used to require constant bit rate, and ogg only had a psuedo constant bitrate. And now it's obsolete because of opus.

AAC only can pack more data in it compared to MP3 at low file sizes, the higher you go, the less this is true.

mp3s been popular since 1997 and more modern formats like aac/vorbis/opus/whatever just offer marginal benefits. plus the encoders are still improving, mp3s today are smaller than before at the same quality
