"How old are you again?" is better, in my opinion. "Again" as in "come again", "tell me again".

"How old were you?" might lead the other person into thinking you're asking his/her age at a specific time or event in the past

I think a way to think about it is the "state" of the information.

So, when you're asking someone their current age, it's about their present state.

But when someone says "What was it" they're referring to their past state where they knew what "it" was.
