First, I find people are more likely to notice you with a phone than with a huge DSLR. The big camera makes it clear you're not hiding, and people guess you're just sightseeing.
It also helps a lot of you have someone to help you. Have him pretend to pose, while you focus on someone behind. I use that method a lot on these...

Also used to use one or those ultra-zoom bridge cameras not too long ago. It made it very easy as you could be anywhere, but image quality was shit at those distances. I'm happier with the DSLR.

Any DSLR will do. I use a 18-200 lens, but something with more reach would be better. At 200 you're at a safe distance, but my best pics are a lot closer. People don't have 360 degree view, so it's not too hard to take them. And you can always pretend to be taking a pic of the landscape and they happen to pass by in front of you...

I've been spotted a few times, though. But never had any problem. People just get suspicious, but if they can't be sure you were shotting them, they won't even come ask.
I do remember because I have the before and after pics. I was shotting some pics of my GF in the beach, then we went to the tower and showed her the pics. And the camera was on the perfect angle to take those in the meanwhile.
