this is on ios 9. so when you call it an icon pack, you must be using a different os. the clock widget is called cataracs, and the theme is called gotham. If you are running ios, i can tell you how to blur the clock widget and how to get the full gotham theme.

Not my kind of wallpaper. Apps are an obscure folder (I'm guessing) which makes you interact with the home screen more than I would want to. Like I keep my camera app the Google search in my home screen for quick access. Also my dialer is on my home screen for quick/emergency access.
