After reviewing the photo, this is more likely herpes, which is quite common among young children. Take to your MD, and wash your hands if you have made contact as some variants can be contagious. Skin cream with antibiotics should clear this up. This shouldn't really cause discomfort for child.

And in future, don't come to Sup Forums for medical advice on kids. Don't take the risk.

Examine your child as there may be other areas, but as this is potentially a contagious condition, it's possible it may have come from contact from elsewhere. There are lots of reasons, but herpes is more commonly seen on face or genital areas, but can be seen on arms and legs also. If you've only just noticed this op, then first stop to your MD who can take a more detailed examination than I can by photo. In my opinion as mentioned, herpes.
