Serious request, not kidding not trolling not baiting. I like films that depict sadism and people's (possibly animal's as well) suffering, but not sexual, definitely not porn. I like films where there is charismatic maniac who's doing or trying to do very terrific things to helpless people who are extremely terrified. It may include full on sadism with pain and death but does not have to. Great example is my currently favorite film – The Human Centipede. It does not have really bloody scenes but the main protagonist is a charismatic crazy man, and people he caught are very terrified and it is shown in the film. Another film I enjoyed was Melancholie der Engel. I didn't really like A Serbian Film because it was sexualized, and not very realistic, it just tried too hard. So I want to ask for suggestions, and one thing I must say, I have seen most of so-called disturbing films like Salò, Martyrs, Hostel and the likes, so you have to recommend me a really rare gem.

Saw definitely seems to fit. Tobin Bell is the only likable character even though he's the villain and there's this weird perversity with him thinking he's helping people.

It's pretty sadistic and disturbing, and there's no gore or sex in it.

I've seen most of Saw films, not really what I'm looking for. If Tusk is similar then it's might not be what I'm looking for.

'The Skin I Live In' is "charismatic surgeon" kino. There is a sexual element but you might get a kick out of it.

I think I'll see this one, heard about it many times. Sexual is not a big problem, I just don't like it when it's the main point.
The Japanese movie Grotesque is the most sadistic one I've seen I think. It is pretty bad though and not very realistic, so I don't think you'll enjoy it.

3 is a dark comedy but there's a graphic scene of a man getting stabbed in the kidney then a guy fucks the kidney hole

Saw them all ofc, only liked first one. You see it's not about grotesque gore, it's rather about convincing story. That doctor from first centipede, his acting was just on point, the story felt kind of real, all actors were acting good. Then when you take other films they usually trying too hard, for instance I got recommended with this I checked the trailer and saw distortions and noises, this supposedly makes the film scary. Not the kind I'm looking for.
