You should still be going to the docs. If infection sets in you'll have legitimate concern over your own safety, not just you're own delusional thought processes.
Not sure if you've been through CBT, but it seems you're at least aware of your delusions. You should make preparations for (self administrative) prescription sedation in case it happens during a clinic visit again.
Neglecting this problem seems like a sure fire way to be committed against your will. Hope the system picks you up before you do something too crazy.

when my care coordinator comes I tell her I'm taking my meds, I shower, brush my teeth and put on clean clothes I only have to appear to be sane and functional for around 20 minutes a week

Get yourself a pair of these from your local drugstore. Just cut the nail till the end of your nail and then pull it out. The hardest part is pulling out the nail. I recommend buying a small pair of needle nosed plyers to pull it out. Then clean your toes with water and disinfect it. Put a bandaid on and cut the ingrown toenail regularly.

Go to the podiatrist, not a normal doctor. Trust me, it's fine, I've went before. You don't feel a thing when they cut out the nail and the pain goes away fast.
Also you're going to have to go to the doctor anyways if you lose your toes, so it's either go to a specialist or the hospital.
