It's still fundamentally done the same way except the cables are underground (because routing cables under roads and buildings is the simplest thing in the world to accomplish) and the operators / switch boards are automated (replacing a person with a few million transistors, why didn't they just do that first? kek). Some intermediary lengths of cables have been replaced with either satellite or wireless sections. Why they didn't just launch those satellites in the 20's is beyond me.
What you're witnessing there is the simplest way of implementing telephony, not the most complicated.
