As someone who is constantly an asshole to everyone in pretty much every sense, a lot of it's in the delivery and consistency.
If you're generally a nice person and then suddenly decide to be an asshole on occasion, people are going to take it incredibly personally, whereas if you're constantly sarcastically deriding everyone people are generally cool with it, because that's "just you."
Additionally, if it's delivered in an offhand/funny way in a brief in/out, or situationally, it's considered sort of transitory and will generally get a good laugh or a shocked reaction, whereas if you have a lot of buildup/take a lot of effort to come up with things/linger and stare for a a reaction people will take it personal.
