I don't think Tony ever really had friends remember that slow motion shot of the crew laughing at Tony's shitty joke. He had people's loyalty (Sil killing that guy who talked about switching sides in "Blue Comet") but I don't think anyone of those people were really his friend (except for Bobby)

Yeah I almost wish the therapy scenes had ended in season 4 when Tony decides to quit and they leave on happy terms. There were a lot of good therapy scenes between that point and the end but that would have been a much better conclusion to the story imo.

A lot of characters mention Tony was different when he was younger. I figure he was surrounded by old school by-the-books types until when the show started. Between Jackie's death, jr's betrayal, and pussy snitching Tony had a swift paradigm shift. Also don't forget the theme that becoming boss changes you.

By the end of the show Tony might of had no friends but that was his fault. He had treated people like shit, and everyone feared they might just be on the receiving end of one of his psychotic episodes. Especially post-horse

Earlier in the episode she was talking with other psychologists about how sociopaths use therapy to not really change anything and continue their immoral behaviour. Melfi felt after all that was said and done she wasn't helping Tony, if anything she was enabling him.

It still was very sudden and out of character. The whole thing happened in one episode with almost no build up.

I kinda agree. I think the reason why it ended was on point (at the end we learn the true goal of therapy for Tony: to be a better mobster, not to be a good person, so he cannot be redeemed at this point, so melfi, finally making the right moral choice, lets him go).
The whole "mobster goes to the shrink" thing is the gimmick of the series, the framing device. Its there for tony to discuss subjects he would normally never verbalice and to give us insight on his complexity. It a sidenote for us to understand whats going on in the show. So maybe dismiss it in one episode is not so bad.
Still, there was no setup for it (inb4 tony character development). I would have liked it more if it was set up better.

I'd imagine Sil, Artie and Jackie at least were his friends as they were a gang when they were kids. But I suppose the power corrupts in both sides, and they started to see Tone as the Don more as a friend.

Most Friendship is pretty fragile and meaningless. Soldiers have bonds that aren't built on friendship and those are much stronger, more akin to family. Friendships on average last 2 years and are products of proximity more than anything. And the internet has commodified the concept almost completely.
The majority of adults have an average of two close friends. Look at your parents, my mom has about 4 very close friends like 25 years plus who are basically family and my dad 1, an army buddy he's not seen in person in at least 6 years.
