On an interview, del toro said that during this scene the director kept giving him the objective of "penetrate", I imagine del toro was doing a few things and they settled on this one. That's really what directors should be doing, it was good to hear.

Point is that positioning is very evocative and was arrived at through experimentation, we don't need to know what happened specifically nor are we meant to.
