Actually, something interesting is that most European towns and cities had an English name at some point that was different from it's original (e.g. Spires), but most are falling out of favor. So I'm sure if you look at old enough mauscripts you'll find strange names for Dunkirk and Berlin even though nowadays everyone just calls them by their original name.

My favourite English name is Leghorn for Livorno in Italy. Kelmorajn too is an old Slovenian name for Koln (am Rhein) used by medieval Slovenian pilgrims.
It's true, though. If you look at manuscripts from England before like 1850 you'll find all sorts of different names, Hell even Shakespeare called Norwegians "Norweyans" (even though that's a denonym and not a toponym, you get my point).
