>walk in, deputy is suctioning the airway while the medic draws up meds and the LT is calling for a helicopter
>blood is everywhere on the floor, like a massive puddle, once I get down and take over suction I can strongly smell blood
>guys wife and daughter are talking to the cop off to the side
>the guy shot himself in the face blowing off his lower jaw
>still alive and conscious
>his airway is full of blood, no amount of suctioning is keeping up
>he takes a breath and inhales large amounts of blood with every breath
>He is awake watching all of us try to save him while he drowns in his own blood
>His jaw bone is in pieces and I can see pieces of jaw bone with teeth attached sticking out of whats left of his lower face and exposed bone marrow
>his tongue is mostly gone
>we RSI (rapid sequence intubation) the guy and load him up
>I ride with the medic unit to the LZ while bagging the guy
>We load him on the helicopter and off they go

A level 4 in a small county thats nearest level 2 trauma center is nearly 60 miles away. Our ER staffing at night is like 3 nurses, a monitor tech and a Dr. with oncall ortho docs. We have imaging and labs but no cardiac or stroke capabilities. Our hospital really is a joke but we get to do cool shit like that because they are shorthanded. They even call out quarters and ask medics to come over and tube if they want (because a lot of these Dr.s don't tube as often as our medics, plus it goes towards recert)

I worked for a hospital EMS service that worked like that. If you weren't on a run you were basically running around the hospital doing lift assists and getting IVs and helping in the ER. EMS would run the in-hospital codes 99% of the time because nobody knew ACLS like the medics, and RNs suck at CPR. But they never made the ER staff leave their patients to work a code.

It took me years to mature to the point where I actually appreciated the cost of me wrecking, instead of the "it's just my life, what you complaining about?" attitude. Had some riding buddies in their 50s who still thought like that though, even the ones that had lost riding buddies. Some just never learn.

I mean, INITIAL GCS was probably 14, but it went quickly downhill from there. Hard to count a RR when he's screaming "I'm dying!" over and over.

The OR was just a quick elevator ride away, and we pumped enough blood in him to get him there, but needless to say there was a red line on the floor and a puddle in the elevator.

The wackiest part was that GI doc flopping those guts out. In the middle of the ER, nonetheless. Of all the traumas I've been in, I've never seen that one. I've had them cross-clamp just to rule out descending aortic injury, but never had them just take a peek.

I'm not that guy, but I am the trauma surg intern. We don't pick who we get to work on. A patient is a patient. I've worked on drunk drivers who left DOAs on the road. When I'm on the floor, I don't care what they did or didn't do... And, honestly, you have to take the 'W's when you can because a lot of my cases are out of my hands before they even roll into trauma.

christ. worst thing ive seen was the immediate aftermath of a suv running over a guy at high speed. i was riding in my friends car passing an area of the road where a car was pulled over and people were standing around. i saw a bunch of blood and meat all over the road and thought "aw fuck. someone hit a deer" then i saw shreds of t-shirt and jeans mixed in with the gore. Thats when I saw the sirens coming in from every direction

before i could tell him, my friend was going "should i loo-no. no i shouldnt look. fuck"

we drove extra careful the rest of the way to the bar. each of us immediately had a shot of whiskey. bartender knew us and that we rarely ever drank and got us to tell him the fucked up story. Stayed there till we were both good and sober, drove back home extra careful.

i was too. kid made it through initial resuscitation but they withdrew care later that day. there was another one that got away and called 911, so it was a rough day in the ER when they showed up. There were pictures on the news of the family and they weren't even recognizable.

Part of embalming is running a formaldehyde mixture through the circulatory system, but that just takes care of the outer tissue, mostly. But yeah. You ram an aspirating rod near the belly button, puncture/drain as much from internal organs as possible, and then fill with chemicals to harden.
