In this scene you can clearly see Vader is not wearing any kind of leash. Was she lying? Did someone make a mistake filming this scene? Mistakes like this really take me out of the movie.

He was holding Vader's leash but it was not attached to Vader at the time and was out of frame. Tarkin dropped it on the floor prior to any moment you see both his empty hands at the same time.
If he put the leash down Vader should've run off.

Alternatively, Leia was holding his leash. She expected to Find Tarkin while she was holding Vader's leash, which again is unatached and out of frame.

It's a pity Lucas didn't make this clearer.
It's also not clear how holding Vader's leash enabled her to more easily find governor Tarkin.
Vader was originally black but they changed him to white when deciding to make him Luke's father.
That's why the leash was retconned in the SE. If you watch the original theatrical version he has a leash around his neck.

It is the truth from a certain point of view. She does not actually say that Tarkin is holding Vader's leash, she is only stating that she should be expecting him to hold his leash.
She was already with Vader. All she had to do is follow the leash and at the other end there would be the leashholder. She expected it to be Sheev, but when she saw Tarkin, she became a bit embarrassed of her mistake and said she should have expected Tarkin to be holding Vader's leash.
that explains why they could cut his head off without legal issues.

One of the Vaders was Luke's father, and the other Vader killed Luke's father (like Obiwan Kenowi said). Luke killed the killer of his father in his cave. Luke's dead father was later revived by Sheev using the dark side of the force.
It's a bit confusing that there are 2 people with the exact same name and clothing in the movies.
Well that explains where Darth Vader went after his ship went spinning off into space at the end of A New Hope. That plot hole always bugged me.
