the only thing that matters is how much money you can make (if it's gdp ppp per capita) and how likely you are to be unemployed.
because for some complex medical procedures like MRI for example you have to wait for almost six months sometimes (yes people do wait for so long, i know lots of such cases). so if someone really bothers you you have to pay for it anyway to get the procedure done quicker
