IIRC the cover was just the first draft and the artist planned on drawing the cover based on the 3D model (a relativly common thing) and the band though it ok like that so they just used it

Bushwick Bill (the midget, a member of the Geto Boys) got shot in the face while his girlfriend was trying to remove a gun from his hands during a PCP and alcohol fueled suicide attempt. The photo was taken as the other two members of the group wheeled him down a hospital corridor. Given that the lead single is an all time classic hip hop track about suffering from mental illness (Mind's Playing Tricks on Me) and the rest of the album is largely about the violence and psychological pain of daily life in the ghettos of Houston, the cover fits perfectly. The way it plays off of the title of the album is beautiful as well. I mean here he is, a poor black midget, shot in the face, drunk, high on PCP and suicidal but he's still making deals on his phone with his crew right beside him looking determined as fuck because they can't be stopped, even by this.
