At the time the book was written men with long hair (assumed to be hippies or queers) were heavily persecuted in rural areas. Local good ole boys would beat the piss out of them and even the cops would get in on the abuse.

One common form of abuse was cutting off their long hair. That is what they were trying to do in Rambo.

The song "Turn the page" was written in the same year as first blood, and it describes the persecution and fear that comes from entering a small town American diner at the time. Looking different was a crime in some parts of the country.
