The book "How To Be Canadian" has a story written in the early 1900s by a journalist from London who is travelling across Canada. It's a long quote, a couple pages, so I'm not going to type it all here but he goes into great detail about how much more patient Canadians are in a train station queue than even the Brits.

Netherlands is interesting because we mostly are queuers but theres always a ragtag group of people who ignore customs and rush into the train before people are out

Both. In the beginning there usually is a small tussle of a small group of people that forms an extension which later turns into a queue over time when the disordered group in the front disappears.
