Monarchism is just the support for a monarch to be the Head of the State, period. It may be hereditary or not, authoritarian or not, have the executive power or not, it really doesn't matter as long as the head of the State is someone who after its election to monarch retains its power and its title till death. An example of monarchy in which the monarchy is neither hereditary is for example the Vatican State, historically Venice was also pretty much an elective monarchy.

Fascism on the other hand is a complete ideology, which has pretty defined ideas on many aspects, from the social structure to the economy and culture (see: autarchy, corporativism, nationalism, authoritarianism).

Monarchy only regards the head of the State, fascism regards the structure of the whole society. The two aren't incompatible.

If fascism is so complete then why does it have so much variance in specific policies and programs between implementations of it?

It can be confusing because monarchies have existed for a long time and have adopted different ideas at various points in history. Especially during the 19th and 20th century when nationalism became more prominent.

Before that, monarchies didn't give a damn about nationalism. Take a country like Austria-Hungary for example. All legitimacy was derived from royal affairs, blood relations and church support and the empire expanded wherever it could.
