Where is it common to say "ciao" out of Italy?
In Germany it's almost as common as the corresponding German words. Unlike Italians we only use it to say good bye.

This Portuguese/Brazilian guy I worked with used to use it on the phone when talking with other Portuguese speakers.
It is also a word in portuguese, but here it means only good bye.
