Why is it that O'neil is able to write about Batman taking on some street gang or some b-list villain and make it as interesting as when other writers do stories that have Gotham being a virtual warzone with an a list villain or some big bad organization?

It's because many writers are unable to make a story interesting if the stakes aren't ridculously high. Scott Snyder is the worst offender when it comes to this. Gotham was basically blown up and on fire for the entierty of his run, it was very, very stupid.
