Subjunctive often implies a possibility and shares it's conjugation in English with the simple past. In practice, half the time in English the verb following "if" is conjugated in the subjunctive, but depending on context it may not be.
The indicative is the mood that carries the significance of certainty and is the mood of the verb following "when" (this is always the case in English).
The conditional "would" therefore should only be employed with the subjunctive and not the indicative
The imperative is the command tone and has an implicit subject, it's irrelevant to the discussion, my bad

The subjective is the "subject" or whatever having an action performed on them or having something change it's current state. If it's past then the event already happened and if it's present then it's happening at the very moment.
The conditional is the actual condition of said subject and how it feels or how it is physically.
